Sudbury Youth Basketball provides a safe learning environment for players of all abilities that develops the athletic skills, character, sportsmanship, & teamwork of Sudbury school children in grades K to 8, by participation in competitive basketball.

Please click on the Coach Evaluation - 3rd-8th Grades 2023 tab found on the left side of this page to access one (1) coach evaluation for the head coach; if there were 2 co-coaches or you would like to fill one out for the assistant coach, please return to this page and fill out an additional evaluation for that coach.
If you have more than one child who played in SYB this season, please return to this page or the K-2 Program eval to fill out additional evaluations.
We do encourage you to provide your name but it is OPTIONAL; we ask for your name so that an SYB Board member can follow up with you to clarify, if needed
All evaluations are STRICTLY CONFIDENTIAL; names are never shared with the Coach(es).
